Section C: Medical Releases

I hereby authorize the Education Director, or person designated by the Director of Education, to obtain emergency medical care for my child(ren) in the event such care is indicated. I give my permission for my child(ren) to receive emergency medical care by any nurse, doctor, paramedic or member of a medical staff of a hospital licensed by the State of California. I understand that every effort will be made to notify a parent/guardian prior to treatment.

I certify that my child(ren) is(are) in good physical health. They have my permission to participate in all activities that are part of the regular religious school program. Field trips may be arranged by Congregation Beth Shalom, and transportation may include bus or vehicle driven by a classmate's parent or guardian.

By typing my name, I confirm I have read, understand and agree to the above.
Section D: Building Community Commitment
Learning Commitment

At CBS we are committed to lifelong Jewish learning and aim to provide all children with this important foundation.  In order to become Bat/Bar Mitzvah at CBS, we expect that children (at minimum starting in 3rd grade) will attend  our educational program in order to achieve their B’nai Mitzvah milestone.  Enrollment in the CBS educational program involves Sunday attendance for children of all grades. Additionally, children grades 4th - 6th grade are expected to participate in weekday Hebrew tutoring to prepare them for their B’nei Mitzvah. If you have concerns about these expectations please contact Benjamin Yudin, Education Director @ educator@cbshalom.org.

By typing my name, I confirm I have read, understand and agree to the above.
Family Engagement

You may have noticed that at the top of the form we begin with family information.  This is intentional.  The work of raising and educating Jewish children cannot be done in the school alone, but is a project of the school, the family and the community. You are not just registering your child(ren) for school today, but in fact your entire family.   When you come in the building and spend time learning and volunteering with us, you make a powerful statement to your children that Jewish learning is a lifelong process and that we are all on the same team in creating these experiences for them.

We have pre-established dates that require family participation such as first Friday of the month Family Shabbat Service which are all indicated in our education program calendar. Additionally, we hope that parents will join us for enrichment activities and other fun congregational events on and off site.

In addition to those opportunities to participate we are in need of regular volunteers to make our school work.  Would you consider supporting our school in an ongoing role or a limited engagement?

Media Release

From time to time your child’s photo may be taken in our classrooms or special events.  We use these photos in the synagogue newsletter (Chai-Lites), on our synagogue display boards, the CBS website as well as private Facebook groups and other publicity materials.

Section E: Payment Information

In Jewish tradition, it is the job not only of parents but of the community to provide education for children. The CBS community is committed to this obligation.  As such, our religious school fee only covers a portion of the cost of our religious school.  Both fundraising efforts like the Purim Carnival and congregational dues heavily subsidize our children’s education.  We are best able to plan appropriately when we know early on how many students to expect in each grade.  As an incentive for you to register early, we offer a discount to families who register and pay online or by mail postmarked on or before Sunday May 31st.  Regular fees go into effect on June 1st.
